PLEASE READ CAREFULLY — THIS IS A RELEASE OF LIABILITY AND WAIVER OF CERTAIN RIGHTS

In consideration for being allowed to participate in any Gaming Ninjas LLC (“Gaming Ninjas”) event or activity, I, the undersigned participant (and/or parent or legal guardian of the listed minor(s)), agree as follows:

1. Acknowledgment of Risk

I understand that participating in video gaming activities inside or around the Gaming Ninjas trailer involves certain risks — including but not limited to tripping, electrical hazards, flashing lights, motion sickness, and possible injury from falls, collisions, or other participants. I voluntarily assume all such risks, both known and unknown.

2. Release and Waiver

I hereby release, waive, and discharge Gaming Ninjas LLC, its owners, employees, contractors, agents, and affiliates from any and all liability, claims, or causes of action arising out of participation in Gaming Ninjas events or use of its equipment, including any injury, illness, or property damage that may occur, whether caused by negligence or otherwise.

3. Indemnification

I agree to indemnify and defend Gaming Ninjas LLC against any claims, damages, or expenses (including attorney’s fees) resulting from my participation or the participation of the minor(s) listed below.

4. Medical Authorization

In the event of an emergency, I authorize Gaming Ninjas LLC staff to seek medical treatment for myself or my minor child(ren) and agree to be financially responsible for any related costs.

5. Media Release

I grant permission for Gaming Ninjas LLC to photograph or record participants during events and to use such images or recordings for marketing or promotional purposes in print, web, or social media, without compensation. (You may opt out by notifying Gaming Ninjas in writing.)

6. Parental Consent (if applicable)

If I am signing on behalf of a minor, I affirm that I am their legal parent or guardian and consent to their participation under the terms of this waiver.

7. Governing Law

This agreement is governed by the laws of the State of Indiana. If any portion is found invalid, the remainder shall remain enforceable.
